Submit a Complaint Regarding Violation of ARDMS Standards
All complaints regarding possible violation of ARDMS Discipline Policies must be received in writing using the Discipline Complaint Form (hyperlink to form). The written complaint should include as much information and detail as possible about the circumstances that form the basis of the complaint, documented evidence of the violation, and the complainant's contact information for ARDMS internal use, should follow up be required.
