Effective September 1, 2009, ARDMS will no longer accept Social Security #’s as an identifier when requesting a school report.

Program directors, in order to receive a school report, you must fill out and submit a Request for School Report Form
*Please note: School reports may only be processed for the current year and/or 3 years prior.

The Request for School Report Form MUST:

• Be sent via e-mail to schoolreports@ardms.org
• Start on the first available line (line 23)
• List formal names only
• Have the date of birth stated numerically, MM/DD/YYYY
• Have at least one (NOT ALL) of the following identifiers for each student listed: an ARDMS ID number OR the date of birth

The Request for School Report Form MUST NOT:

• Be sent via fax or regular mail
• Include nicknames
• Have the month written out in the date of birth

Please send your completed Request for School Report Form to schoolreports@ardms.org.

Please note: If you are requesting data for more than one class, you must send a separate Request for School Report Form for each class.

What You’ll Receive:

An ARDMS school report will include a list of the names of candidates who took and passed an examination, which examination each of the candidates passed during the dates specified, and the credential earned by each listed candidate at such time.

A school report will also include aggregate examination and raw score results, including the percentage of candidates who passed an examination. Individual raw scores will not be included in an ARDMS school report, nor will the names of individuals who failed the examination. ARDMS will not issue a school report unless data regarding at least three candidates who passed an examination can be reported.

Your school report will be sent to you via e-mail to the e-mail address from which the school report was requested. If you would like your school report sent to an e-mail address other than the one from which the Request for School Report Form was sent, please specify in your e-mail request.

There is no charge for this report.

Please allow up to two weeks to receive your school report.
