You need to submit all the required documentation listed under your applied-for prerequisite in order to complete the application process. You will have 21 days from the date you submitted the online application to submit the required documentation. Verify you have all the required documentation by utilizing the ARDMS Application Checklist.

Do not continue if you cannot provide all documentation to ARDMS within 21 days from the date of submission of the online application.

Please Note: If all the required documentation is not submitted within 21 days, ARDMS will automatically refund your examination fee less the $60 processing fee per examination.

A completed ARDMS Clinical Verification form is required as a prerequisite for most applicants applying for a specialty examination. Please refer to the prerequisite chart for specific details.
