Duplicate Score Reports
Scores Earned Prior to 2000:
Requests for score reports for examinations taken prior to 2000 can be processed at a fee of $25 USD (for each score report requested). Please complete the Score Report Form in its entirety. Print or type all information and include a check, money order or credit card for the $25 USD fee (for each score report requested). Please allow 10 business days for the report to be sent via regular mail. The Score Report Form will be returned due to incomplete information, failure to sign the form, or failure to pay the fee.
Scores Earned from 2000 to Present:
Please contact PearsonVue (the ARDMS test administration company) for score report(s). If the examination was taken within one year of the request date, then the processing fee is $10 USD (for each score report requested). If the examination was taken more than one year ago, the score report fee is $25 USD (for each score report requested). Requests must be made in writing, include the candidates name, complete address, examination date, examination name, and ARDMS number. Send the written request and full payment (check or money order payable to PearsonVue) to: PearsonVue, Attention: Duplicate Score Reports, P.O. Box 8588, Philadelphia, PA 19101. Please allow 10 business days for the report to be sent via regular mail.
Examination Score Verification
ARDMS does NOT hand score exams. We can process a score verification at a fee of $35 USD. Score verification ensures that the computer counted the correct and incorrect answers appropriately but, does not review the questions and answers. You must complete the Exam Score Verification Form within 30 days of having taken the exam in question. This form must be completed in its entirety. Please print or type all information and include a check, money order or credit card processing fee in the amount of: $35 USD. The Exam Score Verification Form will be returned for incomplete information, failure to sign the form, or failure to pay the fee. In deciding whether to have your score verified, please consider that ARDMS examinations are scored electronically with a high degree of accuracy. It is therefore unlikely that verification will alter your original score.
