All applicants must have a valid CV form on file in order to be approved to sit for an ARDMS specialty examination (first-time applicants and reapplicants). CV forms are valid for one (1) year from the date of signature, the form is not required for the physical priniciples and instrumentation examinations. If you do not have a valid CV form on file, you will need to submit an updated form with a current date.

There are currently four separate CV forms:

  1. Standard CV form for the listed specialty examinations (AB, OB/GYN, NE, VT, AE, PE)
  2. Breast-specialty examination and breast-specialty clinical experience documentation form
  3. Fetal echocardiography examination
  4. International applicants

Please refer to each form for specific details.

All CV forms are valid for one year following the date of signature. The form is not required for the Sonography Principles & Instrumentation (SPI) examination.

In the rare instance when an applicant may be unable to obtain the required signature from an ARDMS-registered sonographer or vascular technologist certified in the applied-for specialty for the standard clinical verification form, the applicant may send a written appeal to ARDMS providing full detail as to why he or she has been unable to obtain the signature of a qualified sonographer or vascular technologist.

The appeal should consist of a self-generated letter indicating that the applicant is requesting an exception CV form, which specialty that form is for and the reason(s) for that appeal.

ARDMS will consider the following reasons as appropriate arguments for appeals:

1. Local facilities do not have appropriate ARDMS-registered sonographers/vascular technologists and other institutions may be too   far away from the applicant for a reasonable commute.

2. The number of qualified sponsoring sonographers/vascular technologists in the applicant’s region is relatively low.

All written CV signature appeals will be reviewed on a case-by-case basis. Appeals will be considered based on full documentation of the reason for the appeal and the undue hardship resulting from the inability to obtain an ARDMS-registered sonographer/vascular technologist sponsoring signature on the standard CV form in the applied-for specialty area.

If the appeal is approved, the applicant is granted an “exception signature appeal.” A unique Exception CV Form will be sent to the applicant so that an interpreting physician, who has directly observed the applicant perform the applied-for studies and has interpreted the applicant’s scans, may act as a qualified signatory on the standard CV form.
