Test Center Accepted/Unaccepted Forms of Identification

Important Testing Center Update: 

Acceptable Forms of Identification (All ID's must be issued in the English language)

Primary ID (must contain a photograph and signature, not expired)

Note: The test vendor does not recognize grace periods. For example, if a candidate's driver's license expired yesterday and the state allows a 30-day grace period for renewing the ID, ARDMS still considers the ID to be expired.

Secondary ID (signature, not expired)

Sanctioned Countries

The following countries are subject to economic and trade sanctions administered and enforced by the U.S. Department of the Treasury's Office of Foreign Assets Control (OFAC):

Note: If the candidate presents primary or secondary identification from one of the countries, he or she will be required to provide proof of primary residence in the U.S. or another non-sanctioned country.

The sanctions prohibit any U.S. company (including the ARDMS test vendor and its business partners) from exporting goods and services to these countries, or doing business with their foreign nationals.
