Important Testing Center Update: 

• At the test center, you must present two current, valid signature IDs, one of which must be a non-expired government-issued photo ID with  your signature; see the accepted list of IDs here. 
• The name on the application must EXACTLY MATCH the name on both current, valid signature IDs. 
• Jane R. Doe and Jane Rachel Doe DO NOT EXACTLY MATCH. 
• Failure to present two acceptable IDs will prevent your admission to the test center. If this happens, you will be marked absent and you will forfeit the entire examination fee and seat. 
• If the names do not EXACTLY MATCH, update your ARDMS name of record. 
• A candidate is NOT ALLOWED to leave the testing center to obtain their ID’s, and the candidate is NOT ALLOWED to have someone bring them their ID’s while they wait at the testing center.
• All IDs presented at the testing center must be issued in the English language.

Acceptable Forms of Identification (All ID's must be issued in the English language)

Primary ID (must contain a photograph and signature, not expired)

• Government-issued Drivers License
• U.S. Dept of State Drivers License
• U.S. Learners Permit (plastic card only with photo and signature)
• National/State/Country Identification Card
• Passport
• Passport cards
• Military ID
• Military ID for spouses and dependents
• Concealed Firearms License
• Alien Registration Card (Green Card, Permanent Resident Visa)
• Government issued local language ID (Plastic card with photo and signature. ID must be in English.)
• Voter Registration Cards issued by the state government that are plastic and include a photo

Note: The test vendor does not recognize grace periods. For example, if a candidate's driver's license expired yesterday and the state allows a 30-day grace period for renewing the ID, ARDMS still considers the ID to be expired.

Secondary ID (signature, not expired)

• U.S. Social Security Card
• Debit/(ATM) Card
• Employee ID
• School ID
• Credit Card (A credit card can be used as a primary form of ID only if it contains both a photo and a signature and is not expired. "Check Identification" (CID) is not accepted as a signature. Any credit card can be used as a secondary form of ID, as long as it contains a signature and is not expired. This includes major credit cards, such as VISA, MasterCard, American Express and Discover. It also includes department store and gasoline credit cards.)

Not Acceptable Forms of Identification

• International driver's licenses that are not issued by a government
• Naturalization papers
• IDs from countries subject to U.S. Sanctions (below)
• Voter Registration Cards that are paper, or plastic and do not include a photo
• Marriage License
• Divorce Decree
• Amended Documentation
• Birth Certificate
• Name Change Decree
• Health Insurance Card
• Ontario Health Insurance Program (OHIP)

Sanctioned Countries

The following countries are subject to economic and trade sanctions administered and enforced by the U.S. Department of the Treasury's Office of Foreign Assets Control (OFAC):

• Cuba
• Syria

Note: If the candidate presents primary or secondary identification from one of the countries, he or she will be required to provide proof of primary residence in the U.S. or another non-sanctioned country.

The sanctions prohibit any U.S. company (including the ARDMS test vendor and its business partners) from exporting goods and services to these countries, or doing business with their foreign nationals.
