Applicants must have a valid Clinical Verification (CV) Form on file in order to be approved to sit for an ARDMS specialty examination (first-time Applicants and Re-applicants). CV Forms are valid for one (1) year from the date of signature. If you do not have a valid CV form on file, you will need to submit a new form with a current date. The CV form is not required for the Sonography Principles & Instrumentation (SPI) examination.
Standard Clinical Verification forms are intended for Applicants residing in the U.S or Canada. International Clinical Verification forms are intended for Applicants residing outside the U.S. or Canada. Please note that the home address on record at the time of application submission is the basis for determining which form to use.

In the rare instance when an applicant may be unable to obtain the required signature for the standard CV form, the applicant may send a written appeal to ARDMS providing full details as to why he or she has been unable to obtain the signature of an ARDMS-registered sonographer or vascular technologist certified in the appropriate specialty. The appeal should consist of a letter from the applicant requesting an exception CV form for the appropriate specialty, and the reason(s) for that appeal.
ARDMS will consider the following reasons as appropriate arguments for appeals:
1. Local facilities do not have appropriate ARDMS-registered sonographers/vascular technologists,
and other institutions are too far away from the applicant for a reasonable commute
2. The number of qualified sponsoring sonographers/vascular technologists in the applicant’s
region is relatively low.
All written CV signature appeals will be reviewed on a case-by-case basis. Appeals will be considered based on full documentation of the reason for the appeal and the undue hardship of obtaining an ARDMS-registered sonographer/vascular technologist sponsoring signature on the standard CV form in the appropriate specialty area.
If the appeal is approved, the applicant is granted an “exception signature appeal”. A unique Exception CV Form will be sent to the applicant so that an interpreting physician (must hold a current, valid license to practice in the U.S. or Canada), who has directly observed the applicant perform the necessary skills and has interpreted the applicant’s scans, may act as a qualified signatory on the standard CV form.
